The collision occurred at 2:32 p.m. on the eastbound 210 Freeway near Ayala Drive.
A white work van and a white Acura collided.
The events leading to the crash are unclear.
Hernandez was a passenger in one car.
She was rushed to Arrowhead Regional Medical Center and died there.
