Juniata County, PA (August 13, 2025) – A fatal motor vehicle accident (MVA) occurred on August 13, at approximately 5:00 a.m. on State Route 35 in Mifflin Borough. The crash involved a 2007 Chevrolet Express van traveling northbound, which swerved into the southbound lane while attempting to make a right turn.
According to Pennsylvania State Police, the van struck two curbs and sidewalks before crashing into a guardrail and coming to a stop. Manuel Vazquez-Perez, 30, of Sunbury, was sitting in the backseat and was not wearing a seatbelt at the time of the crash. He was later pronounced dead by the Juniata County Coroner’s Office after being airlifted to Mifflintown Airport.
The driver and two other passengers, also not wearing seatbelts, sustained minor injuries. They were treated on-site before being transported to a nearby hospital.
Emergency response teams, including Mifflintown Fire Company, Central Juniata EMS, Port Royal EMS, Fayette EMS, and Thompsontown EMS, were on the scene. An investigation is ongoing, and no further details about the cause of the crash or the identities of the other individuals involved have been released.
