HILO, Hawaii County, Hawaii, Oct. 11, 2026 — An elderly man was left in critical condition after a head-on collision Friday evening, October 9, on Ponahawai Street in Hilo. A woman driving the other vehicle suffered injuries described as non-life-threatening.

Hawai'i Island police said officers responded at 8:37 p.m. to a report of a two-vehicle crash on Ponahawai Street between Komohana and Kapiolani streets. Investigators said a 2016 Toyota Tacoma pickup headed mauka (upslope) on Ponahawai Street reportedly crossed the centerline and struck a 2013 Toyota Corolla heading makai (downslope) in the opposite direction.

The 77-year-old man driving the pickup was found unresponsive at the scene, according to police. He was taken to Hilo Benioff Medical Center, where he remains in critical condition. The 59-year-old woman driving the sedan was also taken to Hilo Benioff Medical Center and was being treated for injuries not considered life-threatening.

Police said they do not believe speed, alcohol or drugs were factors in the crash. The East Hawaii Traffic Enforcement Unit assisted at the scene, and the collision remains under investigation. Officers are asking anyone who witnessed the crash to contact Officer Brian Kohara at the Hawai'i Police Department, or to report information anonymously to Crime Stoppers.

Our thoughts are with those who were injured, and we wish them a full recovery.

Why Head-On Crashes Do So Much Damage

A head-on crash is unlike most other collisions because the speeds of both vehicles add together at the moment of impact. Two cars each moving at 40 mph do not strike with the force of an 80 mph hit, but the combined closing speed is still far higher than what either vehicle would experience in a rear-end or side-impact crash, where one vehicle is often stopped or moving in the same direction as the other.

Vehicle design also plays a role. Modern cars are built with front crumple zones meant to absorb energy gradually in a frontal hit, but those zones are engineered around a certain range of impact speed. When closing speeds exceed that range, the crumple zones compress fully and the remaining force transfers into the passenger compartment. A mismatch in vehicle size or weight, such as a pickup truck meeting a sedan, can also mean the heavier vehicle transfers more of that force into the lighter one.

According to the National Highway Traffic Safety Administration, when a passenger car and a light truck collided head-on in a fatal crash, an occupant was nearly three times more frequently killed in the passenger car in 2023. Seatbelts and airbags reduce the risk of serious injury in a head-on impact, but they work within limits tied to how much force the structure around the occupant can absorb before buckling.

Frequently Asked Questions

What typically causes head-on collisions?

They often happen when a vehicle drifts or crosses the centerline, due to distraction, falling asleep, a medical event, or losing control on a curve or slick road.

Why do head-on crashes tend to cause more severe injuries?

Because both vehicles are moving toward each other, their speeds combine at impact, producing more force than a crash where one vehicle is stationary or moving in the same direction.

How can drivers lower the chance of a head-on crash?

Staying alert, avoiding distraction, keeping a safe following distance on two-lane roads, and slowing down on curves or in poor visibility all help a driver stay within their lane.

Where do head-on crashes happen most often?

They are more common on two-lane roads without a median, where opposing traffic is separated only by a painted centerline.
