Sullivan County, IN (May 30, 2026) – A fatal roadway accident on US 41 near the junction of Old Hwy 54 claimed the life of a Sullivan man late Friday morning.
According to preliminary findings, the collision occurred at approximately 11:00 a.m. when a 1998 Ford Ranger driven by 59-year-old Bradley D. Galenski of Sullivan was traveling southbound in the northbound lanes of US 41. Multiple 911 callers reported the pickup moving in the wrong direction before it struck a northbound 2026 Freightliner.
Investigators said the Freightliner was operated by 71-year-old Clarence Mackey of Alapaha, Georgia. Mackey attempted to avoid the oncoming vehicle but was unable to prevent the head-on wreck. The force of the collision sent the Ford Ranger off the roadway, where it rolled onto its side. Galenski was ejected from the pickup, while the semi crossed the median and came to rest in the southbound lanes.
Galenski was pronounced dead at the scene. His family has been notified. Drug and alcohol testing remains pending as part of the ongoing review.
Investigators continue to examine the factors that led to the fatal crash. Our heartfelt condolences are with Mr. Galenski’s family and friends during this difficult time.
What Happens During a Wrong-Way Driving Investigation?
Wrong-way driving collisions often require a detailed review because they can develop quickly and involve severe outcomes. Investigators typically begin by examining travel direction, vehicle positions, roadway markings, and any available witness accounts. Traffic camera footage, dash camera recordings, and 911 call records may also help establish a timeline.
Vehicle inspections are commonly conducted to determine whether mechanical issues contributed to the sequence of events. Testing may also be performed to identify whether impairment or other factors played a part. Road design, signage visibility, and weather conditions are frequently reviewed as well.
In cases involving commercial trucks, investigators may examine electronic driving records and vehicle data systems. The information collected helps create a clearer picture of how the collision occurred and may assist in identifying the factors that led to the wrong-way travel.
