Las Vegas, NV (June 12, 2026) – A passenger died following a two-vehicle accident on Warm Springs Road near Montessouri Street in southwest Las Vegas on Wednesday afternoon. The crash occurred at approximately 5:13 p.m. on Wednesday, June 10, west of Rainbow Boulevard. According to the Las Vegas Metropolitan Police, a 2020 Toyota Sienna and a 2021 Tesla Model Y were both traveling eastbound when the Toyota crossed into westbound lanes before veering back into eastbound traffic, leading to a collision.

A passenger in the Toyota was transported to University Medical Center with serious injuries. Hospital staff initially reported her condition as critical but stable. However, her condition worsened later, and she died from her injuries. The Clark County Coroner’s Office identified her as 33-year-old Loreta Hayashi of Las Vegas.

Both drivers remained at the crash site. The driver of the Toyota was later taken into custody after investigators reported signs of impairment. No additional injuries were confirmed. The investigation into the circumstances of the crash remains ongoing.

We extend our sincere condolences to the family and loved ones of the victim.

Why Lane Departure and Impairment-Related Crashes Can Be Severe

Crashes involving lane departures usually result in severe outcomes because vehicles may cross into opposing traffic at higher speeds with little warning. When a vehicle moves into oncoming lanes and then re-enters traffic, drivers may have limited time to react and avoid a collision.

Impairment can further increase crash risk by affecting judgment, reaction time, and lane control. These factors can contribute to unpredictable driving patterns that place other road users at risk.

After serious collisions, investigators typically examine vehicle paths, roadway markings, and driver condition to determine how and why a vehicle left its lane. These findings help reconstruct the sequence of events leading up to the impact.
