South Bend, IN (November 14, 2025) – A deadly head-on crash late Wednesday night in South Bend left one man dead and two others seriously injured, prompting an investigation by local authorities. The collision happened at approximately 10:15 p.m. on Mayflower Rd near Belleville Dr, on the city’s west side.
Initial reports indicate that a 48-year-old woman driving an SUV northbound had just exited the bypass when a southbound pickup truck veered into her lane. The pickup, driven by a 63-year-old South Bend man, collided head-on with the SUV. Emergency crews responded swiftly, transporting both drivers and a 58-year-old male passenger from the SUV to a nearby hospital with serious injuries.
Authorities have since confirmed that the passenger, identified as James Barnes of South Bend, succumbed to his injuries. The current medical condition of the two drivers has not been publicly released.
Members of the St. Joseph County Fatal Crash Team are leading the investigation to determine the cause of the deadly impact. Officials have not disclosed whether impairment, speed, or mechanical issues played a role. Updates will follow as the inquiry continues.
We extend our deepest sympathies to the family and loved ones of James Barnes and hope for a full recovery for the surviving victims.
Head-on Collisions in Indiana
The tragic incident on Mayflower Rd highlights the devastating consequences head-on collisions can have, particularly when they occur at high speeds or on major commuter routes. In this case, investigators believe the pickup truck unexpectedly entered the opposite lane just moments before impact, a scenario that is often associated with drowsy driving, distraction, or loss of vehicle control.
South Bend’s west side, especially around the Mayflower Rd corridor, experiences frequent traffic from drivers accessing the nearby bypass and residential areas. When traffic flow is disrupted by driver error or unexpected lane departures, it leaves little time for oncoming motorists to react. Head-on crashes are among the most dangerous types of accidents in Indiana and consistently result in high rates of severe injury and fatality due to the forces involved.
In light of Wednesday night’s crash, officials are again emphasizing safe driving practices, including maintaining lane discipline, reducing speeds during nighttime travel, and avoiding distractions. Road design, traffic volume, and driver attentiveness all play critical roles in preventing tragic collisions like this one.
