Washington, UT (May 28, 2026) – A man has died after a two-vehicle head-on crash at the intersection of Rio Virgin Dr and Washington Dam Rd on Wednesday afternoon, May 27. The man was driving a vintage Ford Model T when the fatal sequence unfolded.
Reports state the Model T was turning off Rio Virgin Dr onto Washington Dam Rd when the driver attempted a U-turn and entered the eastbound lanes instead of continuing west. A F-450 pickup truck towing a roughly 15-foot flatbed trailer entered the intersection as the car moved into its path.
Both vehicles met in a head-on, sideswipe contact near their front driver-side fenders. The Ford driver was ejected, and the vehicle rotated 180 degrees before coming to a stop.
Emergency units provided life-saving efforts, but the man died at the site. The inquiry into what led to the deadly head-on remains ongoing, and additional information may be released as it becomes available. Our condolences go out to the victim’s loved ones.
