Middletown, OH (February 4, 2026) – A serious accident on Monday evening in Middletown resulted in the death of one driver and left another person with severe injuries, according to local authorities. The collision occurred just after 6 p.m. on February 2 near the intersection of Carmody Blvd and Hook Dr, an area known for steady evening traffic.
When Middletown police arrived at the scene, they discovered that one of the drivers involved had suffered fatal injuries and was pronounced dead at the location. The individual’s identity was not immediately released. The second driver, who was the only occupant of the other vehicle, sustained serious injuries and required transport to a nearby hospital for advanced medical care.
Paramedics and fire crews worked quickly to assist the injured driver while securing the area to prevent further hazards. Investigators confirmed that only the two drivers were inside the vehicles at the time of the accident.
