Polk County, OR (July 4, 2026) – Four people were injured in a crash involving a Polk County ambulance at the intersection of Highway 99W and Hoffman Road.
Emergency units responded at about 6:39 p.m. on Friday, July 3, after a collision involving an ambulance and another vehicle. According to Polk County Fire District No. 1, the ambulance was en route to an emergency call in Monmouth when the crash occurred. Officials also stated the ambulance had recently cleared the scene of a separate fatal motorcycle crash in Independence and was not transporting any patients at the time.
Three fire personnel inside the ambulance and the driver of the other vehicle were taken to a hospital in Salem; however, officials said all four sustained minor injuries. Still, the Oregon State Police is handling the investigation into the crash.
Our thoughts are with those injured as they recover.
What Is Typically Reviewed in Emergency Vehicle-Involved Crashes?
When an emergency vehicle is involved in a crash, investigators typically examine speed, right-of-way, and whether lights and sirens were active at the time. They also review traffic signal timing, intersection visibility, and driver response from surrounding vehicles.
Because emergency vehicles frequently operate under urgent conditions, dispatch logs, GPS tracking, and onboard data can be important in reconstructing the timeline. Physical evidence, such as vehicle damage, skid marks, and intersection layout, also helps determine how the collision occurred. These findings help clarify how the crash developed and whether any contributing factors were present.
