Fort Wayne, IN (March 13, 2026) – Authorities have identified the woman who died following a crash on Thursday morning, March 12, in the 2300 block of West Jefferson Boulevard in Fort Wayne.
Officials said 27-year-old Mikaela Tish Forker of Kendallville died from multiple blunt impact injuries after the collision. The crash happened shortly before 8:30 a.m. near the SweetCars dealership.
According to preliminary information, a white car traveling west on West Jefferson Boulevard crossed the center line and struck an eastbound vehicle head-on. Both drivers, adult women, were taken to a local hospital after the impact.
Forker later died from her injuries at the hospital. The other driver was reported to be in stable condition with minor injuries.
