Ephraim, UT (September 8, 2026) – A passenger died Monday, September 7, after two vehicles collided on State Route 89 near milepost 305.
According to the Utah Department of Public Safety, a northbound Honda Civic crossed double yellow lines while attempting to pass another vehicle. The Civic then struck a southbound Toyota Tacoma.
The passenger inside the Honda suffered fatal injuries. The drivers of both vehicles were taken by ambulance to nearby hospitals for treatment.
The circumstances surrounding the fatal wreck remain under investigation as officials work to determine what led to the collision.

Why Are Double Yellow Lines Used on Two-Lane Roads?
Double yellow center lines are used to separate opposing traffic and generally indicate that passing is not permitted. They are commonly placed where road design, visibility, curves, hills, or other conditions make crossing into oncoming traffic particularly risky.
A driver who crosses the center line can enter the path of vehicles traveling in the opposite direction. This can leave very little time for either driver to avoid a head-on accident, particularly when both vehicles are moving at highway speeds.
Passing decisions can also be affected by limited sight distance. Even when a driver can see traffic ahead, the available distance may not be enough to complete the maneuver safely. Staying within the designated lane and waiting for a legal passing zone can reduce the chance of a dangerous encounter with oncoming traffic.
