Greene County, OH (August 18, 2026) – The Ohio State Highway Patrol is actively investigating a wrong-way head-on crash that left four people hospitalized early on Sunday morning at SR 4 at Adams Road in Greene County. The collision occurred around 2:50 a.m. on August 16.
The preliminary investigation found that a 27-year-old man was driving an Acura TLX south in the northbound lanes of SR 4. A 41-year-old woman was traveling north in a Chrysler Town and Country when the Acura struck the Chrysler head-on. The Acura driver was taken to Miami Valley Hospital with minor injuries. His 29-year-old male passenger suffered serious but non-life-threatening injuries and was also hospitalized.
The Chrysler driver and her 27-year-old female passenger were trapped inside the vehicle and required mechanical extrication. Both women were taken to Miami Valley Hospital with serious but non-life-threatening injuries. The Acura driver was found at fault and determined to be impaired at the time of the crash.

Why Are Wrong-Way Crashes Especially Dangerous?
Wrong-way crashes can be severe because vehicles traveling in opposite directions may collide head-on.
