Institute, WV (April 22, 2026) – Two people died and multiple others required medical care following a chemical incident Wednesday, at a facility near Institute in Kanawha County.
The incident occurred around 9:30 a.m. at Catalyst Refiners, a silver recovery business. Emergency units responded after a violent chemical reaction took place during the cleaning of a tank ahead of decommissioning. The reaction involved nitric acid and another substance, leading to the release of hydrogen sulfide gas.
Two employees were pronounced dead as a result of the incident. At least 19 others required treatment, including decontamination and evaluation, with one person reported in critical condition. Seven emergency responders were also evaluated at a hospital following possible exposure. Some individuals at the scene declined treatment. Authorities continue to investigate the cause of the incident, and more details will be released as soon as they become available.
Our condolences go out to the victims’ loved ones.
How Chemical Reactions in Industrial Settings Can Become Dangerous
Industrial facilities often handle chemicals that can react under certain conditions, especially during maintenance or cleaning processes. When substances like acids interact with other materials, they can produce gases or heat that may quickly become hazardous. These reactions can happen unexpectedly if residues remain inside tanks or equipment.
Gases such as hydrogen sulfide can pose serious health risks. Exposure levels and duration can affect how the body responds, with symptoms ranging from irritation to more severe effects like breathing problems or unconsciousness. Because some gases may have strong odors, workers might notice them quickly, but high concentrations can still overwhelm safety measures.
Emergency response in these situations often includes evacuation, decontamination, and medical evaluation. Proper handling procedures, ventilation systems, and protective equipment are key factors in reducing risks during industrial operations.
